Superior Health Care Providers, Inc. is committed to providing superior quality, safe and cost-effective nursing, rehabilitation and supportive services in your home. As a health care provider, we will ensure that your privacy and dignity will be respected.
This mission is guided by the following:
- Provide home health care services regardless of race, color, age, religion, nationality or disability.
- Provide patients with superior quality care alternatives to hospitalizations.
- Provide skilled intermittent rehabilitation services, nursing care, and personal care to enable patients to remain at home during recuperation from illness, accident, surgery or disability.
- Assist patients and their families with the transition from the hospital to home through cooperative planning and service coordination with hospital staff, physicians, and services in the community.
Collaborate with the referring physician to assure the best possible patient outcome, given the patient’s potential and the avail-ability of resources.
